The Survey Plan Submission to Surveyor General form is used to submit all plans prepared pursuant to the Land Act, the Mineral Tenure Act and the Coal Act and well site plans prepared pursuant to Oil and Gas Activities Act to the Surveyor General for filing into the Surveyor General's vault and the Crown land registry.
The Survey Plan Submission to Surveyor General form must be electronically signed by a BC land surveyor. See Obtain a Digital Certificate from Juricert.
How to Complete the Form
Open a blank copy of the Survey Plan Submission to Surveyor General form from inside Acrobat
From the pull-down menu, select File > Save As and save the form under a suitable directory and file name
To make it easier to see the data-entry field on the forms, we recommend turning on Acrobat's field highlighting feature. See Data Entry Tips.
In Section 1 (BC Land Surveyor Identification), enter the following data, pressing the Tab key to advance from field to field:
- Name of the land surveyor followed by the firm name if desired
- Street address, line 1
- Street address, line 2 (if necessary)
- City
- Province
- Postal code
- Additional instructions or information (enter your telephone number, email address, and your own file reference number)
Fees for the submission of plans to the Surveyor General (referred to in the LTSA fee schedule as Parcel Designations and Plan Examinations) will be deducted from the land surveyor's myLTSA Enterprise account.
Unless the survey plan has been prepared specifically for a Provincial Government Ministry the Deduct LTSA Fees ? must be selected and the word Yes should appear beside the checkbox.

Users who often enter the same information into this section may find it useful to create a profile of commonly used data. If a profile already exists, clicking Import Profile will enter the default information stored in the profile. This information is editable. See Export Reusable Data.
In Section 2 (Plan Identification), select from the drop down, the Act under which the plan is being submitted. The default is the Land Act.
Depending on the plan type you select, the form will display different sets of fields. The following headings provide instructions for each Act.
Land Act
Enter the plan number (and if necessary, the commission number of the land surveyor who requested the plan number if different from the land surveyor completing the survey). Select a survey type from the list:
- Survey Plan
- Posting Plan
- Compiled Plan
- Partially Compiled Plan
- Control Survey Plan
- Former Indian Reserve Survey
- Partial Park Boundary
Coal Act
Enter the plan number (and if necessary, the commission number of the land surveyor who requested the plan number if different from the land surveyor completing the survey). The form will display a survey type of Coal Lease.
MTA Title (1-10 cells/units)
Enter the plan number (and if necessary, the commission number of the land surveyor who requested the plan number if different from the land surveyor completing the survey). Select a survey type from the list:
- Mineral Title Survey (by statute)
- Mineral Title Survey (by client request)
- Placer Title Survey
MTA Title (11-20 cells/units)
Enter the plan number (and if necessary, the commission number of the land surveyor who requested the plan number if different from the land surveyor completing the survey). Select a survey type from the list:

- Placer Title Survey
MTA Title (21-30 cells/units)

MTA Mining Lease Reduction
Enter the plan number (and if necessary, the commission number of the land surveyor who requested the plan number (if different from the land surveyor completing the survey)). The form will display a survey type of Mining Lease Reduction Survey.
Oil & Gas Activities Act
Select a grid ("Petroleum & Natural Gas" or "Peace River Block"), and complete the fields under Location (the exact fields displayed depend on the Grid you have selected):

- P&NG Block
- P&NG Group (three fields)
- Land District
Peace River Block

- LS (Legal Subdivision)

The data in the grids is the basis for the Wellsite ID, which will appear on the form when viewed from the View All Packages page. See: View Packages
If you selected "Oil & Gas Activities Act" under Section 2, the words "Not Applicable" are automatically entered in Section 3.
In Section 3 (Parcel Identification and Legal Description of Land), enter the following data, pressing the Tab key to advance from field to field:
- PIN (Parcel Identification Number): This will have been supplied by the Surveyor General Services when a parcel designation was provided or survey instructions provided
- Area of each parcel surveyed
- Legal Description: As supplied by the Surveyor General Services
To enter information on more than one parcel, click Use Schedule. Acrobat will append an extra page to the form (Additional Parcel Information), with space for information on up to eight parcels. The words "See Schedule" will appear and a schedule will be appended to the form. Any text already entered in the parcel information will be carried forward to the schedule. Once the schedule is selected, the legal description field cannot be changed and either the schedule must be completed or a new form started.
To enter information on more than eight parcels, click Additional PIN Pages at the foot of the schedule.
In Section 4 (Certification), enter the completion date of the field survey and plan, plus the ECR number and the date the ECR was completed
In Section 5 (Alteration), do not select the Alteration checkbox unless the submission is an alteration to an electronic plan already submitted.
If the plan is being re-submitted with an amendment (see Submit Alterations as a Result of a Defect Notice by the Surveyor General), click Alteration and enter the following information:
- the Control Number on the previously submitted Survey Plan Submission to Surveyor General form
- a description of the alteration made to the plan, or to the form
If the description requires more space than is available on the form, click Use Schedule. Acrobat will append an extra page to the form (Additional Description of Non-material Alteration).
The words "See Schedule" will appear and a schedule will be appended to the form. Enter the entire description in the schedule; don't split the description between the main form and the schedule.
If the description requires more than one page, click Additional Pages at the foot of the schedule page.
In Section 6 (Plan Data), select the Yes or No checkbox to indicate whether the plan is a re-survey of an existing Primary Parcel.
If appropriate, enter the name of the agency driving the survey, with their Agency File Number. You can type in the agency name, or select one of the values from the dropdown box.
To enter information on more than one agency, click Additional File Numbers. Acrobat will append an extra page to the form (Additional File Numbers Schedule), with space for 24 additional agencies.
Enter the following information:
- Survey Method ("Field Survey" or "Compilation")
- Scale (e.g. enter "5000" for a 1:5000 plan scale)
- Surveyor's reference or file number
In Section 7 (Explanatory Notes), enter any explanatory notes. Explanatory Notes might contain an explanation of how the cadastral structure was re-constructed in a complex situation or might contain a record of specific direction provided to the land surveyor by the Surveyor General or staff. This section replaces the cover letter submitted with a Mylar plan. If the explanation requires more than one page, click Additional Pages.
Attach the Plan Image to the Survey Plan Submission to Surveyor General form. See: Insert Pages in a Form.
Apply your Electronic Signature. To affix the electronic signature, click the box to the right of the certification statement.
The incorporation of the subscriber's signature into an electronic plan is the subscriber's certification that the information contained in each notation, endorsement, statement, or certification made by the subscriber and set out in the electronic plan is correct.
Once an electronic signature is affixed, the form cannot be modified in any way. It is advisable to save an unsigned copy of the form before affixing an electronic signature. For detailed procedures on affixing an electronic signature see Apply an Electronic Signature.
